Quality Improvement Partnership

Application

If you are interested in participating in the Quality Improvement Partnership, fill out the application and either email it or mail it to us!

Applications for our Quality Improvement Partnership projects are reviewed and compared to availability of mentoring services.  If there are more applications than available services, applications will be prioritized based on the following criteria:

1. Licensor request, assistance needed with licensing violations or issues
2. Retention of provider, provider is in danger of closing businesses without assistance
3. Parent request
4. Access to less resources, low-income provider or no other services are received at program
5. Geographically isolated provider
